SECTION ONE

Fees, Requirements and Pre-Camp

Introduction

One of the important aspects of preparation for camp is making sure that all requirements have been fulfilled. These requirements include:

  • paying all tuition and fees
  • submitting an accurately completed Camper Evaluation and Personal Information Form
  • having your child examined by a licensed physician and submitting an A.C.A. Camper Medical Form, completed and signed by that physician, and signed by you.

You will find both forms enclosed in the package along with this manual. Please take a few minutes to read through the following sections, to avoid any complications or delays.

Fees

All camper tuition and fees (first month, second month, full season) must be paid according to the following schedule:

January 1: One-half of the tuition is due
April 1: Tuition balance - Paid in full

An unpaid account may be considered as an automatic cancellation and the reservation may be given to the next camper on our waiting list. A bus boarding pass will be issued only to those campers whose accounts have been cleared. Our office will be sending out reminder notices under separate cover.

Please avoid complications. Clear your tuition balance at your earliest convenience. If you have any questions about tuition, please call us. We'll be happy to help.

To ensure proper and prompt credit for your payment, please include your account number -- which can be found on your bill -- with your payment. Do not send cash in the mail. Payment can be made via personal check, certified check or money order -- made out to Camp Moshava -- by mail or in person. If you intend to pay in person at our camp office, we suggest that you call in advance to make sure that someone will be available to help you.

Cancellations

All reservations, cancellations and changes of reservations must be in writing. Any change requested via the telephone can not be honored. Please be aware that no refunds of tuition are made for children who return home voluntarily during the summer, or for children who are sent home for disciplinary reasons. This is clearly stated on our camp contract. All refund requests must be made in writing to the Camp Office. The Refund Subcommittee of our Camp Committee decides on refund requests within two months after the end of the camp season.

Many parents have inquired about cancellation/interruption protection to cover Program Costs. We make available a competitively priced program that we believe meets the needs of our families. We highly recommend you review this comprehensive protection plan created especially for the Camp/Summer Study industry.

     CAMP/SUMMER STUDY PROTECTION PLAN

Complete details of the plan and enrollment forms are available online at TravMark. Purchasing the plan is simple, after reviewing the plan provisions; you need only to do the following:

  1. Complete the simplified enrollment form including the following information:
    Organization Name:Camp Moshava
    Organization ID#: camo11
  2. Print the enrollment form (an insurance certificate will be included with the enrollment form); and
  3. Mail the enrollment form (keep the insurance certificate) along with your check payable to Trip Mate.
    It’s as simple as that!

The Protection Plan provides coverage for:

  • Program Cancellation - (protects your non-refundable payments if you cancel program before it begins) for covered reasons such as injury, sickness, terrorist acts and transfer of employment of 250 miles.
  • Program Interruption - (protects your non-refundable payments if you interrupt your program) for covered reasons such as injury, sickness, terrorist acts and transfer of employment of 250 miles.
  • Medical Expenses/Emergency Assistance - medical bills for up to one year and for any special transportation required for medical reasons including expenses for parent to visit if hospitalized 3 days.
  • Baggage - for damage, lost, or stolen personal articles.
  • Medical Records On Line - at your option, instant access to your medical records is available with the plan.

Please be advised that some important coverage enhancements are time-sensitive and require that your premium be paid by April 1st, if your initial deposit for your program was made prior to April 1st, or within 30 days of your initial deposit if made after April 1st.

Additional Information:

  1. If the plan is purchased by April 1st (or within 30 days of your initial trip deposit, if made after April 1st ), the plan will include coverage for Terrorist Acts as defined in the policy and Exclusion for Pre-Existing Medical Conditions will be waived (provided you are not disabled for travel at the time our premium is paid).
  2. If the plan is purchased after April 1st or more than 30 days after your initial trip deposit (if your deposit is after April 1st), but before final payment is made, the plan will not include coverage for Terrorist Acts and the exclusion for Pre-Existing Medical Conditions will not be waived.
  3. Once paid, the premium is non-refundable.

The plan is offered and administered by Trip Mate. If you have any questions regarding this coverage, please call Trip Mate, the Plan Administrator, at (800) 888 7292 (8am-5pm CST) and refer to Plan ID#550.

Camper Evaluation Form

Camp Moshava's educational philosophy embraces the importance of individual attention and program personalization. The Camper Evaluation/ Personal Information Form is an tool to help our staff prepare for your child. This confidential form is designed to enable us to help your child become an integral part of camp life, and adjust to general and specialized activities as smoothly and easily as possible. Any requests for bunk assignments, tutoring and individualized attention should be indicated on this form. While we can not guarantee fulfillment of all requests, we do our best to honor as many as we can. You should fill out this form as accurately and completely as possible. Under no circumstances should you permit your child to fill this form out. Your complete and honest answers to all questions will greatly facilitate our ability to care for your child.

Please return the form to our New York office by April 1.

Medical Form

All children must be examined by a licensed physician prior to their arrival at Camp. Enclosed in the package along with this manual you will find an American Camping Association health form. ALL sides of the form must be completed and returned to our New York office by June 1.

You will notice that the first three sides of the form are to be completed by you as a parent. The last side must be completed and signed by a licensed physician. Make sure to sign the section of the form authorizing the Camp Moshava doctor to render medical care. No child can be allowed on a bus or permitted to enter camp without a properly completed and signed medical form on file with the camp.

Therapy

With increasing frequency, it has become perfectly natural for children to see a psychotherapist or counselor to assist with growth, development or adjustment issues. If your child is, or has been in therapy, please provide the information requested in the appropriate section of the personal information form.
